Full-Time Hospice Spiritual Care Coordinator/Chaplain

Monday - Friday, 8:00 AM to 5:00 PM

 

Overview:

The Hospice Spiritual Care Coordinator is responsible for the provision of spiritual care services to patients, families and caregivers of Signature Hospice, either directly or through coordination of care with other spiritual counselors on an as needed basis.

 

The Hospice  Spiritual Care Coordinator will be dedicated to the Mission, Values and Care Commitments of Signature Healthcare at Home and embrace and demonstrate our mission: “To Enhance The Life Of Every Person We Serve.”

Responsibilities Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.Provides direct spiritual care to patients and families/caregivers.Serves as liaison and support to community chaplains and spiritual counselors. Provides consultation, education, and support to the interdisciplinary group on spiritual care.Actively participates in quality assessment performance improvement teams and activities.Provides for funeral or memorial services for patients as Accepts and performs other related duties and responsibilities as required. 

 

 

Qualifications/Skillsets

 

Graduate of accredited seminary or school of theology (BO, MDiv or equivalent theological degree preferred). Experience working with death and dying, individuals/family/caregiver.Hospice/ experience Possesses and maintains CPR certification.

 

 

Ability to work as member of interdisciplinary team/group.Comfort in an interfaith setting.Ability to accept different lifestyles, cultures, beliefs, and values.Ability to network with community clergy and congregationsKnowledge of and commitment to hospice philosophy of care.Working knowledge of state and federal hospice regulations Excellent verbal and written communication skills.Ability to travel to all business locations.Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
