Summary of Responsibilities:
Under the direct supervision of the B ridges to Success Mentor Manager , the BTOS Mentor assist in coordinating, implementing, and monitoring of assigned services and activities offered through the Bridges to Success (BT O S ) program component of the Focus on Self Sufficiency Department (FSSD). The BTOS Mentor will provide support to program participants seeking economic mobility and self-sufficiency through program participation. This position will work closely with the FSSD Team in providing additional supportive resources and counseling services to consumers . The BTOS Mentor serves as a staff liaison that connects consumers , with resources and opportunities that promote personal, educational, vocational, & financial development , and self-sufficiency. The BTOS Adult Mentor works in the capacity of a “life-coach” to help facilitate positive behavioral changes that help consumers take ownership in transitioning into greater independence , and sustainability.
The BTOS Mentor works closely with the FSSD/BTOS teams to ensure that consumer enrollment, retention, and service and program outcome goals are achieved. Additional duties include: outreach and recruitment; intake & screenings; consumer assessment ; diagnostic testing and interpretation; development of individual service plans and goal setting; case management, coordination, & conferencing; crisis intervention; mediations; financial literacy counseling and planning; life-skills and self-sufficiency skills training and coaching; information & referral; facilitation of individual counseling and group meetings; transportation assistance; advocacy; literacy & developmental training and workshops; TASC, TABE test preparation assistance and referrals ; data management and reporting; resource development; coordination of special events; collaborative & partnership development; evaluation & service monitoring to ensure compliance with objectives, standards; and to promote consumer and program growth.
The BTOS Mentor will also assist consumers with navigating appointments, applying for compensations, and connecting individuals to resources and services that promote self-sufficiency, advancement, and sustainability. The BTOS Mentor actively participate s in the development and modifications of the program operations and research.
The BTOS Mentor is also responsible for maintaining assigned data-base and informational systems, record keeping and reporting duties. This includes data input ; consumer and program documentation ; log sheets ; report submissions ; and preparing related c ommunications and documentation sources, to assist in monitoring activities and progress toward goals.
The BTOS Mentor dedicates time towards developing beneficial partnerships with various community sectors and key community stakeholders, in mobilizing community and advocacy efforts, especially for vulnerable populations. As assigned, this position also assists in special outreach efforts, including implementing special events, to ensure that the visions of the Agency/program are achieved; and to support positive outcomes for all consumers and families.
Qualifications / Education and Experience:
Education : Bachelor’s Degree preferred, Associates degree required in Social Work, Counseling, Mental Health, Human Services, or related discipline . An equivalent combination of education, related certifications and work experience will be considered .
Experience : A minimum of three (3) years of related work experience is required which included: case management, service coordination, consumer assessments, coaching/ counseling, asset development, community outreach, information & referral services, and/or related duties ; preferably in a not-for profit setting .
Must possess good organizational and planning skills; and be attentive to details . Requires good administrative, analytical, negotiations, evaluative and problem - solving skills . M ust be able to manage multiple tasks with tact and diplomacy ; and manage multiple priorities with fixed deadlines . R equires flexibility and ability to work in fast-paced environment; and must be able to implement appropriate changes when needed . Requires the ability to work independently , as well as a member of a work team.
Must have experience and knowledge of effective service delivery strategies including outreach, intake, motivational interviewing, screening , assessment practices, consumer engagement, and service coordination. Requires experience with goal setting, s ervice planning, and navigation al services. Requires good judgement and decision-making skills; the ability to recognize support networks; and the ability to collaborate with individuals in promoting self-sufficiency and personal development.
Demonstrated experience and knowledge of best-practices in case management , case conferencing , counseling, asset building, life-skills, and mentor relationships, is required. F amiliarity with crisis-management and/or disaster-case management approaches is a plus. Experience with both in-person , remote case conferencing, zoom practices, and other modes of consumer engagement, distance learning , etc. is a major plus.
Experience in educational/career/vocational development is strongly preferred. S hould have some familiarity with the following: local occupational, educational, and labor market information/laws, vocational testing and career interest assessment/inventory instruments and resources .
F amiliarity with Civil Service exams, high school equivalency, testing process is helpful . K nowledge of financial literacy, credit establishment/ repair and banking processes preferred . Job placement and knowledge of educational, workforce initiatives (i.e., Rochester Works), business community/employment opportunities are strongly preferred.
Requires excellent communication skills, oral & written . Should have experience in preparing and disseminating marketing, informational & educational materials, and related communications to consumers. Requires ability to interpret and present on program activities, data, and reports . M ust be able to conduct public and community presentations and related meetings, as assigned. M ust have the ability to initiate and maintain partnerships with both internal & external customers and organizations.
Requires good public speaking, presentation , and facilitation skills . Some classroom management and small group facilitation experience is preferred. K nowledge of group dynamics, consumer engagement, integration activities and team building s t rategies is strongly preferred. Should have experience in developing and presenting life-skills and asset development training/instructional /informational sessions incl.; educational/vocational prepara tion, financial literacy, etc. R equires experience coordinating, scheduling , and implementing instructional sessions and supplemental meetings and special events.
Must have strong interpersonal and customer service skills; and the ability to maintain a high level of confidentiality for all items of a sensitive nature. Requires enthusiasm, and the ability to work with diverse individuals and groups and organizations from diverse backgrounds, cultures, and in a variety of settings. Should possess a particular understanding of the issues relating to special needs, low-income, and/or vulnerable populations; and sensitivity to issues faced by individuals and families moving from dependence on public assistance, to independence and self-sufficiency.
Bi-lingual capabilities preferred: English and Spanish preferred
Requires the physical health and physical capability to work in an office and classroom setting.
Requires a valid NYS Driver’s License, and access to reliable transportation.
