Duties and Responsibilities + Complete assignments to support resident service plans and Centers' programs in accordance with Center, Division and contract requirements. + Document all relevant shift activity in 'log book' according to Center policy. + Review resident log daily and document resident activities daily. + Assist in conducting telephone and walk-in intakes for prospective residents; make referrals to community resources and services. + Provide medications in accordance with resident's medical chart; document in resident chart. + Monitor residents' behaviors; report incidents/violations orally and in writing to Program Manager. + Prepare and distribute meals for residents; maintain cleanliness of kitchen and dining areas on assigned shift to include but not limited to dishwashing. + Perform housekeeping duties in resident rooms, including but not limited to cleaning, vacuuming, wiping, sanitizing, and preparation of vacant resident rooms. Serve as a mentor to Family Support Workers. Provide initial and ongoing training as required. _________________________________________________________________________________ Job Qualifications Education, Training, and Licensure/Certification + Training or college courses in social work or human services preferred. + Required to attain and maintain Food Handler's Basic certification.
