Position Perks & Benefits: + 29 Days of PTO + Competitive Pay + Eligibility for HRSA Loan Repayment *eligibility requirements + Employee benefits package - health, dental, vision, retirement, life, & more** + Competitive 401(k) Retirement Savings Plan – up to 5% match for Part-Time and Full-Time employees + Company-paid basic life insurance + Emergency Medical Leave Program + Flexible Spending Accounts – healthcare and dependent child-care + Health & Wellness Program + Employee Assistance Program (EAP) + Employee Discount Program + Mileage Reimbursement Key Responsibilities: + Assist and Coach Families: Assist and coach families to understand their child’s needs and build helpful relationships with child and family team members. + Resource Provision and Support: Provide resources (videos, books, articles, etc.) and moral support during crises. + Meeting Preparation and Participation: Help with meeting preparations and participate in activities defined by the treatment team and family. + Individualized Service Plans: Develop individualized service plans and participate in staffing for continuity of care. + Outside Referrals: Make or assist with outside referrals for issues beyond the treatment milieu. + Intake and Representation: Assist with intake processes and professionally represent the agency. + Documentation: Document all services provided according to standards. + Crisis Intervention and Group Education: Provide crisis intervention when necessary and facilitate group education sessions. + Professional Development and Support Activities: Obtain professional development training and support activities like mutual support groups and recovery dialogues. + Parent/Caregiver Assistance: Assist parents/caregivers in coping with a child's serious emotional disturbance and model resilience. Offer individual or group services with an educational and support focus, sharing lived experiences and promoting recovery tools. Help parents/caregivers recognize their resilience, connect with community support, and make independent choices in their child's treatment. + Policy and Conduct Adherence: Adhere to system policies and the Code of Conduct. + Health Service Systems Understanding: Demonstrate understanding of health service systems and assist in securing resources for healthy living. + Assessment and Plan Development: Conduct assessments to develop individualized family plans. Engage families in the wellness process, advocate in agency and community settings, and facilitate wraparound meetings. Monitor family plans, provide support and coaching, and identify options to meet family needs. + Resource Presentations and Wellness Activities: Facilitate presentations on local resources, assist families with health management skills and self-advocacy, and plan wellness activities. + Multidisciplinary Team Participation: Serve as a member of a multidisciplinary team, ensuring the implementation of family plans and collaboration with community resource providers. + Other Duties: Perform all other duties and responsibilities as assigned. Education and/or Experience Qualifications: + High School Diploma or GED required. + Must be at least 21 years of age. + Must meet state qualifications to obtain the Family Support Provider credential. Additional Qualifications: + Thorough knowledge of case management and rehabilitation methods related to mental health services. + Knowledge of challenges faced by individuals with mental illness and how to access community resources. + Familiarity with methods and legal requirements related to mental health and substance use disorders. + Some knowledge of training techniques in a mental health/substance use setting. + Ability to evaluate services and communicate ideas clearly and effectively, both verbally and in writing. + Capability to work with individuals facing multiple challenges and establish effective relationships.
