Job Overview:

The role at Children s Specialized Hospital will assist in efforts to collect information directly from families regarding their social needs/barriers/issues. The Family Navigator Lead will also oversee the work of other Family Navigators who will collectively review patient data and identify appropriate ways to identify and assist families in need while maintaining strict confidentiality of the information collected of patients and their families.

Qualifications:

Required:

HS Diploma or Equivalent

Preferred:

Strong knowledge of computer software packages (Microsoft Office particularly Word and Excel)Proven work experience as a team leader or supervisor strongly preferredPrevious hospital or physician s office experience preferred but not required

Scheduling Requirements:

Part -Time Day 30 hrs weekly8:30am 3:00pm Hybrid schedule

Essential Functions:

- Responsible for supervising, managing, and motivating team members on a daily basis

- Establishes relationships with external community partners in an effort to support CSH patients and families

- Sets clear team goals, monitors team performance and reports on metrics

- Works independently with other departments to develop protocols and policies

- Discovers training needs and provides coaching

- Communicates social needs/barriers/issues to a multidisciplinary team at the hospital in an appropriate manner

- Collects information regarding social needs/barriers/issues directly from families

- Effectively engages and establishes helping relationships

- Identifies and provides resources to families while maintaining patient confidentiality

- Reviews patient data and identifies appropriate families to contact

- Acts as an information and emotional resource to patients and families

- Provide general information as requested, and direct families appropriately

- Monitors and obtains accurate information for data collection, and assist with data analysis

- Other tasks assigned by management

- Hybrid work schedule and will need to occasionally travel between CSH sites
