Providing professional exercise guidance and cardio/pulmonary risk factor modification, with individualized patient centered care that optimizes health and wellbeing, enhances quality of life and minimizes risk of recurrent cardio/pulmonary events that lead to readmission, poor outcomes, and increased health care costs.
Phase I Cardiac Rehab provides Pre Op education, Progressive Ambulation, Lifestyle Modification, Goal Setting, Smoking Cessation, Diet Education, Phase II Referral, and Discharge Education to a very diverse patient population - Coronary Artery Bypass Surgery, Valve Replace/Repair, Percutaneous Intervention, Myocardial Infarction, Heart Failure, Left Ventricular Assist Device, Video Assisted and Open Thoracotomy, Peripheral Artery and Vascular Disease, Structural Heart, Pectus Excavatum, Heart Transplant
