Discharge Planner

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

Associates degree in nursing or human services field required. Bachelor's degree in social work or human services field preferred. One or more years’ experience in a clinical setting preferred. One or more years’ experience in discharge planning preferred. Crisis Prevention and CPR certifications (offered on site) Ability to work with male and female adult (18+) patients with chemical dependency and mental health disorders Teamwork and excellent customer service

ESSENTIAL FUNCTIONS:

Based on patient's needs, develop a continuing plan that promotes their long term health and relapse prevention. Responsible for setting discharge appointments, including group or individual therapy, psychologist or psychiatrist. Make arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care. Ensure all documentation regarding patient discharge planning is clear, concise and timely.
