The Home Health Manager is responsible for leading, planning, organizing, staffing, directing, analyzing and evaluating the services of the Home Care department in order to provide effective home health services that meet practice and quality standards. The Home Health Manager is accountable for home care residents/patients through the nursing process of assessment, planning, intervention, implementation and evaluation. This position also effectively interacts with clients, family members, responsible parties, and other health team members with maintaining standards of nursing.
Responsibilities Provides administrative, therapy and clinical leadership for home care staff to ensure the delivery of quality care that meets the resident’s/patient’s needs.Maintains knowledge base of current standards of practice.Directs the hiring, evaluation, and counseling of home care staff.Oversees services as they relate to direct participant care.Develops, implements, and evaluates the home care department quality assurance program.Serves as a liaison with other departments and residents/patients and families.Performs assessments, planning, implementation and evaluation of client needs and continuity of care.Documents according to current standards and policies.Establish and implement the participant care plan for individual participants.Promotes the Benedictine Mission and Core Values of Hospitality, Stewardship, Respect and Justice by bringing the Mission and Core Values into the day-to-day activities of the company. Current state licensure as a Registered Nurse, which must be in good standingCPR certifiedValid Class A Driver’s License
