Diabetes Educator- Full-Time, 32 hours and Day Schedule 

This is a hybrid role, working at Providence Medical Group Clinics and has traveling requirements throughout the Portland metro area

The diabetes educator provides education for clients with diabetes of all ages according to the individualized needs of the client. This process involves a health and education assessment, development, implementation and evaluation of an education plan, taking into account psychosocial, economic, and language barriers. Duties are performed in a classroom or individual outpatient setting in collaboration with a team of RNs & RDs. The candidate accepts professional development and community outreach as an integral part of professional practice

Required qualifications:

Upon hire: Oregon Dietitian License  Upon hire: National CDR Registered Dietitian - Commission on Dietetic Registration  3 years of experience in treatment and education of diabetes within the last 2 years. Experience with current adult learning theory in relation to diabetes education. Experience with the clinical management of diabetes.

Preferred qualifications:

Coursework/Training and Formal coursework or credentials in principles of adult learning theory and patient-centered learning. Certification in Diabetes Education, or willingness to obtain CDCES certification (within 3 years of hire) Experience with diverse ethnic groups.
