Dementia Community Care Specialist-Full Time

Community Care Specialist
Under the supervision of the Clinic Manager, this position works to develop a supportive rapport and assist patients and their caregivers to access health care and community resources and navigate through the healthcare continuum. Functions as a member of the patient’s collaborative care team to coordinate care and address care plan preventative health and wellness goals. Works collaboratively with the patient to foster self-management and compliance to their clinical plan of care. Works collaboratively with physician, hospital, and community resources to support the patient's clinical plan of care.  
Schedule

Generally scheduled Monday-Friday, 8am- 4:30pm

Qualifications 

Bachelor’s degree in public health or related health service field. Knowledge of health care system and community resources available.Experience with data collection and reporting.General knowledge skills in the use of computers including basic computer applications.BLS certified for the healthcare provider or obtained within three months of hire.
