DayHab Services Facilitator Location: New Windsor, NY, United StatesDate Posted: Mar 31, 2025 Share: share to e-mail Apply Now Save JobSaved Description Location: New Windsor, NY Pay Rate: $23.00 per hour Hours: Monday-Friday 8:00AM-4:30PM (40hrs) OVERVIEW OF PRIMARY RESPONSIBILITES: Day Habilitation Services Facilitator (DHSF) supports the needs of the day hab team, stakeholders and colleagues who provide support to adults with intellectual/ developmental disabilities in collaboration with the Day Services Supervisors and Assistant Directors/ Directors. DHS Facilitator supports the administrative functions of the day habilitation program ensuring compliance with state and OPWDD regulations as well as agency policies and procedures. DHSF streamlines interviewing, hiring, onboarding and training for all day hab staff. DHSF assists with ensuring continuous coverage to maintain staffing ratios at program sites. PRIMARY FUNCTIONS: Duties and responsibilities vary in intensity and frequency: + Supports day habilitation settings with individuals with varying diagnoses. + Collaborates with Human Resources, Day Service Supervisors and Assistant Directors/ Director to promote staff recruitment (i.e. - job fairs, open interviews), screening, and interviewing, selection, and retention, training and professional development. + Maintains applicant-tracking system and follows applicants through the hiring process to ensure timeliness of hire. + Supports employees with finding new positions in order to increase retention. + Tracks direct care staff’s training due dates to ensure compliance with no lapse and provides reminders as needed. + Supervise per-diem staff and assist in scheduling per diem DSP’s. + Completes staff performance agreements, which are aligned with Core Competencies. + Provides performance feedback and initiates progressive discipline as needed. + Completes recruiting related documents for staff actions. + Provides support in covering program sites in absence of Day Service Supervisor. + Participates in meetings as deemed necessary in order to provide support to Day Services Supervisors (i.e. - family, staff, life plans, etc.). + Coordinate intakes and admissions in collaboration with the Day Services Supervisor and Team Leaders. + Monitor, reports and resolves incidents and compliance issues in a timely manner. + Assists with coverage issues and provides essential coverage as needed. + Utilizes technology to review and approve staff timesheets, ensuring all hours are accurately charged. + Provides onboarding to all new hires, implements training programs; trains others as required and attends departmental, agency and relevant community meetings. + Complete POM’s as assigned. + Represent day hab at the Agency meetings + Liaison for community volunteer sites + Provide billing oversight to ensure compliance with regulatory requirements. + Complete stay and interviews for the day hab program and share feedback to create change + Identified as an eVero Administrator to assist with setting up accounts, running reports, approving billing, etc. + Other duties as assigned. ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT: + Actively involved in committees, work groups, and CQL activities. QUALIFICATIONS AND ATTRIBUTES: + Ability to effectively communicate + Have computer skills including experience with Excel and the ability to create spreadsheets + Possess excellent interpersonal skills + Ability to function as a member of a multidisciplinary team + Possess sensitivity to the service population’s cultural and socio-economic characteristics + Ability to organize, multitask and prioritize duties and responsibilities efficiently + Ability to work independently + Must maintain current license/ certification/ training + Possess a valid driver’s license, work eligibility status and have an excellent driving record EDUCATION AND EXPERIENCE: + At least 5 years’ experience in the field and at least 1-year supervisory experience in any field OR + Associate’s degree with 3 years’ experience in the field and at least 1-year supervisory experience in any field. OR + Bachelor’s Degree in any field with at least 1-year experience in the field and at least 1- year supervisory experience in any field, PHYSICAL CHARACTERISTICS: + These physical demands are representative of the physical requirements necessary for an employee to perform the job's essential functions successfully.
