Day Program Manager
Overview A Day Program Manager will be responsible for overseeing all intakes and discharges for the day program, day to day operations, activities and supervision of the Day Supports Habilitation Technicians. The Day Program Manager will also serve as a Qualified Professional/ Clinical Supervisor of the consumers that are serviced by the Day Program. Responsibilities Consumer Responsibilities Ensure Consumer Rights are upheld by the agency and all representatives. Direct line of communication for consumer/families, staff and Care Coordinators. Attend and be an active participant of all Plan of Care meetings and Team meetings and coordinate meetings with families, Case Managers, and staff as needed. Provide Clinical input to meet consumer needs. Review schedule and service authorizations with family and notify family and Care Coordinators as needed regarding changes. Review and complete family packets with families annually. Review Individual Service Plans and revisions for all consumers in the Day Program for agency coherence to stated interventions. Create/implement documents to support consumers by the use of schedules, cards, etc. Review data/documentation and goals met for all consumers in the Day Program. Scheduling staff and completing monthly consumer calendars per current service authorizations. Update calendars with all staffing updates. Complete and/or review incident reports as needed. Lead daily group meeting with all consumers and staff Staff Responsibilities Provide onsite supervision for all direct care staff working on your cases per the current supervision contract. (Consumers are to be seen at least quarterly.) Complete all paperwork required for supervision: checklist, supervision note, data corrections needed, etc. Complete/Review client specific competencies, and other agency policy changes/additions with staff. Ensure that all staff have a working knowledge of the ISPs of consumers they work with. Ensure staff are adhering to all agency policy and procedures and consumer plan of care. Train and interview staff as needed. Conduct monthly staff meeting Meet monthly with program coordinators Administrative Responsibilities Answer the phone and respond to emails within 24 business hours. Instill Agency Philosophy Attend Clinical Leadership Team meetings Case Review and Follow up Follow all LHS policies and procedures Coordinate transportation for consumers being transported to Lindley College via LHS routes Review checklists, facility walk throughs, MARs and other facility based responsibilities Support coordinators as needed to complete schedule for LC that aligns with pillars of the program Qualifications Must meet the minimum requirements of a Qualified Professional: A bachelor's degree in a human services field (i.e. Psychology, Rec. Therapy, Child Development, Education) and 2 years of full time, supervised experience in the field or A bachelor's degree in a field other than human services and 4 years of full time, supervised experience in the field. *Day program experience is highly preferred. Knowledge Requirements: Have a working knowledge of Innovations IDD and developmental disabilities. Current Day Supports Service Definitions under the Innovations IDD Manual Documentation requirements per the Records Review Manual. Ability to draft, train, reviews, and maintains Departmental Policy and Procedural needs. Working knowledge of licensure and DHSR regulation
