Counselor PT - ON1024 - Thurs, Sat & Sun 10AM -6PM Job Details Job Location Elmont - Elmont, NY Education Level High School Salary Range $17.91 - $17.91 Hourly Job Description Provides supportive supervision to residents in the Apartment Program, in their performance and in their daily living skills. Collaborates with residents on assigned goals and their achievements. This position may have supervised / unsupervised contact and or conversational interaction with consumers while working in the residences, day treatments and habilitation facilities. essential Responsibilities + Provide direct care services to resident, including planning and implementation of goals and training, which will develop and enhance activities of daily living skills. These include but are not limited to: + Budgeting + Grooming + Nutritional planning + Use of public transportation/travel training + Use of community resources/community integration + Shopping/menu planning + Housekeeping + Sexual development/socialization skills + Physical health maintenance + Assertiveness/self advocacy training + Monitor resident’s progress. Tasks essential to those duties include, but are not limited to: seeing, hearing and observing residents, performing manual tasks, reading and writing (documentation, lifting, climbing and descending stairs, traveling, embark and disembarking agency vehicles) and utilize public transportation (bus, subway) use of telephone to make and receive calls. + Responsible to coordinate evacuation and ensure residents safety in emergency situations (i.e., fires, fire drills). Tasks essential to these duties are the ability to exit in emergencies, which may require the ability to climb and descend fire escapes and assist others to do the same. + Medication Management and Training ‑ monitor supervise and record residents’ self-administration of medication. + Responsible for crisis intervention requiring availability to escort resident to various clinics or hospital. + Maintain required written documentation including maintenance of resident charts, weekly progress notes, which shall reflect the residents active participation in goal related activities. + Delivering weekly financial allowances to residents and monitoring apartment budgets through appropriate distribution and use of tokens, food, laundry, recreation monies and resident rebates. + Notifying all incidents to supervisory staff within 24 hours. + Reporting of apartment maintenance needs to supervisor. + Working with residents of implementation and towards achievement of monthly and quarterly goals. + Escort residents to required appointments as determined by the Case Manger. + Attend all scheduled staff meetings, mandatory in‑service trainings and area meetings. + Participate in scheduled supervision with Case Manager. + Must be able to transport self to various sites + Required to participate and attend meetings/trainings at various sites that require following different route of travel to work‑site. + Able to lift/carry packages and boxes of supplies. + Ability to count/add/subtract/manage money. Communicate information with all staff and supervisor(s) verbally and in writing for the purpose of giving instructions, providing feedback and adjusting work practices. Other Responsibilities + Participate in activities of other staff members in their absence or during periods of staff shortage. + Act as a role model to residents and staff. + Ability to work flexible schedule as work schedules and location are subject to change. + Be punctual for scheduled apartment visits or make arrangements for coverage with supervisor. + Maintain confidentiality at all times. Job Qualifications High School Diploma or equivalent required. New York State driver’s license and access to a vehicle required.
