Position Description:
Reviews all incoming INR results and doses patients medication based off these results and the parameters set forth by the patient’s provider. Communicates directly with providers regarding critical out of range results. Reviews patient records for changes in condition documented by provider that may affect the efficacy of Coumadin. Adjust dosing accordingly in conjunction with the patients provider. Provides initial and ongoing education to patient and / or family member. Acts as resource for the Coumadin LPN, giving instruction as necessary. Communicates with outside agencies, patients and family members. Other duties as assigned.Requirements:
Education/Certification/Licensure Current Licensure as a Registered Nurse in the State of New York. Current Basic Life Support certification.Experience/Skills/Proficiencies: Bachelor’s Degree of Nursing from an accredited nursing school, preferred. Knowledge of medical terminology, anatomy, physiology and pathophysiology. Familiarity with health care systems, regulations, policies, and functions. Understanding of documentation standards. Knowledge of equipment, supplies, and materials needed for medical treatment. Works well in a team setting and demonstrates strong cooperation skills. Skill in reading and following written and oral medical orders. Skill in initiating appropriate emergency procedures. Skill in handling a number of tasks simultaneously. Experience in a physician office, preferred.
Pay Range: $56,000 - $75,000 DOE
