POSITION SUMMARY: 

 

The Coordinator, Wellness Program Mobile Services is an exciting opportunity to expand Project Open Hand’s mobile services that increases the ability for clients to access services in their communities. This popular service model is at a critical growth point and this role will take the lead in developing and implementing a sustainable growth plan for POH’s mobile services. The Coordinator’s primary responsibilities include a blend of administrative and direct service duties to support the Wellness Program and Project Open Hand’s mission. The Coordinator will play an integral role in managing POH’s Mobile Services program, distributing POH’s medically tailored meals and groceries in neighborhoods throughout San Francisco with the possibility to expand into Alameda County. The Coordinator leverages data to identify trends that inform quality assurance and quality improvement approaches to best serve Project Open Hand clients. The Coordinator will support the Manager, Wellness Program (MWP) as needed to ensure contractual and organizational goals are being met.  

QUALIFICATIONS REQUIRED: 

Three years’ experience providing direct services in Social Service environment and to populations that may be experiencing mental health illness, physical health conditions, and/or substance abuse disordersBachelor’s Degree in Human Service field or equivalent experienceProficient in MS Office, using databases, and internet researchStrong organization skillsExcellent communication skills, verbal and writtenFlexible schedule with the ability to work in POH’s Oakland office At minimum a professional working proficiency in SpanishValid California driver’s license and excellent driving record

QUALIFICATIONS DESIRED:  

Experience using a trauma-informed and client-centered approach to provide services to a diverse community of clients, especially individuals living with chronic health conditions, mental health issues, and/or substance abuseExperience working in a nonprofit client services/community engagement-based settingMental Health or Social Work ExperienceServSafe Certification with demonstrated knowledge of safe food handling practices, sanitation, and hygiene or ability to complete training upon hireExperience supervising staff and/or volunteers Experience supporting the building and/or growing of a public health programBilingual in Cantonese, Mandarin, Russian, Tagalog, or Vietnamese

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: 

Manage all operational aspects of POH’s Mobile Services program, including but not limited to: supporting pack out of meals/groceries; and supporting set up/break down of site food distribution; submitting inventory requests to Grocery Center Operations department; work closely with Grocery Center Operations and Distribution departments to coordinate schedule and inventory needs.Manage programmatic functions pertaining to Mobile Services inclusive of providing case management to promote client engagement and maintain client files as needed on site or via follow-up; submitting client participation reports as needed; Establish and maintain policies and procedures for the Mobile Services Program, including tracking decisions affecting procedure changes and writing/updating policy and procedure manualDocument, analyze, and report on trends related to quality assurance and ongoing quality improvement of the POH Mobile Services program; provide oversight to program including but not limited to: establishing and maintaining goals, staying updated on contract changes, tracking progress toward meeting deliverables, helping troubleshoot barriers in meeting goals, providing MWP with data for invoices, monitoring visits, and delegating tasks to staff and volunteers as appropriateIdentify appropriate partners and resources to design and implement opportunities for Mobile Services program growth.Help to monitor and resolve client and staff incidents, including: reporting necessary information to the MWP, gathering information, facilitating staff discussion regarding intervention, ensuring adequate documentation, writing correspondence, and notifying staff of intervention and resolution. Utilize a client centered, harm reduction model to provide case management to clients that includes but is not limited to: maintaining client caseload as assigned, conducting intake interviews for new and returning clients, assessing and verifying program eligibility, enrollment and assessment of client needs, tracking client adherence, supporting with adherence to POH services, facilitating and follow up on referrals Ensure proper staffing coverage for assigned contract and department needsCoordinate, facilitate, and participate in weekly case management staff meetingsAble and available to make decisions and answer questions in the absence of the MWPRepresent POH at community and outreach events and meetings as needed and appropriate.Caseworker duties as neededOther duties as assigned

Work environment: The noise level in the work environment varies from moderate to loud. The work environment will be ever changing with new clients, volunteers and staff members all sharing space and working together in the administrative offices and POH Grocery Center
