To learn more visit https://www.doctorshosplaredo.com JOB SUMMARY: The Program Coordinator is responsible for program assessment, planning, organizing, implementation, performance improvement, education, community outreach and pre-hospital education regarding sepsis of all patients utilizing evidence based practice guidelines. The Program coordinator works in conjunction with a multi-disciplinary team to provide clinical, and professional expertise to patients, families, and staff members, while acting as the facility's liaison with EMS and EMDS to promote quality in clincal practice for a specific patient population ESSENTIAL JOB FUNCTIONS: Coordinates and oversees the program in collaboration with the Program Medical Director Maintains responsibility for knowledge and skill of clincal requirements in nursing specialties Provides accurate and timely data to the appropriate committees and identifies opportunities for improvement that complies with the fscility Performance Improvement Plan. Collects, analyzes, and oversees the reporting processes of data collection. Responsible for the initial and ongoing education of the hospital staff and development of contents in compliance with mandatory standards, and needs assessments. Evaluates patient outcomes, and reports findings in Midas or refers for Peer Review. Reviews, develops, and manages policies, procedures, guidelines and other matrix resources per standards. Attends meetings, committees, conferences etc. to maintian knowledge and competency in the management of the septic patient. Works in conjunction with Quality to adhere to core measure requirements, data abstraction, chart auditing, and maintaining deadlines.
