Community Support Specialist CPST

 

HeartShare St. Vincent’s Services (HSVS) empowers youth and families to overcome the challenges of homelessness, family crises, addiction, mental illness and poverty. Founded in 1869 as St. Vincent’s Services in Downtown Brooklyn, we began as a home for “poor and friendless boys.” In 2014, HSVS was formed through an affiliation with Heart Share Human Services of New York. Through the affiliation, HSVS developed into a multi-program nonprofit agency with four divisions: Foster Care & Preventive Programs, Integrated Health Services, Housing & Residential Services and Youth & Community Programs. Across our agency, staff members serve our clients with compassion, respect and in collaboration with our outside partners. HeartShare is looking for a Community Psychiatric Support and Treatment (CPST)  in our Foster Boarding Homes:

A Community Psychiatric Supports and Treatment  will assist the youth with the objective of incorporating therapy goals into daily life with focus on Self Care, Coping Skills and Managing Emotions.  A CPST will coach and support youth and provide resources extending therapy for behavioral health while providing ways of helping people with mental illnesses or substance use disorders. Providing counseling and more specialized psychotherapies seek to change behaviors, thoughts, emotions, and how people see and understand situations. This person will provide services within a variety of permissible  locations including but not limited to: Community Locations, where the youth or individuals lives, works, learns or socializes. The worker will provide services to help individuals with behavioral health diagnosis to achieve stability and functional improvement in the following areas: daily living, finances, housing, education, employment, personal recovery and/or resilience, family and interpersonal relationships and community integration.

 

 

Responsibilities:

The worker will provide mobile treatment and rehabilitation services to individuals who have difficulty engaging in site- based programs who can benefit from off-site rehabilitation or who have not been previously engaged in services, including those who had only partially benefited from traditional treatment or might benefit from more active involvement of their family of choice in their treatment.
