Key Responsibilities:

Collaborate with clients, families, and other stakeholders to develop and implement individualized treatment plans tailored to each client's specific needs and goals.

Provide community-based mental health services, including assessment, diagnosis, treatment planning, and intervention implementation.

Utilize strengths-based approaches and evidence-based practices such as Motivational Interviewing, Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), and Trauma-Informed Strategies.

Coordinate care with service providers, including primary care physicians, psychiatrists, schools, and community organizations, to ensure comprehensive support.

Facilitate PSR (Psychosocial Rehabilitation) group sessions to support clients in achieving treatment plan goals.

Maintain accurate, timely records of client interactions and progress in compliance with organizational policies.

Develop and maintain relationships with community stakeholders to expand support networks for clients.

Participate in ongoing training and quality improvement initiatives to enhance service delivery.

Perform additional duties as assigned to meet client and organizational needs.

Education and/or Experience Qualifications:
There are several ways to qualify for this position:

Bachelor’s degree in Psychology, Social Work, Sociology, Education, Criminal Justice, Family Studies, Counseling, Recreational Therapy, Human Services, Human Development, Child Development, Gerontology, Behavioral Science, Rehabilitation Counseling
OR

Associate of Applied Science in Behavioral Health Support degree from an approved institution
OR

Four years of qualifying work experience in a related human services field, such as case management, residential support, developmental disability support, or vocational rehabilitation
OR

A combination of education and related work experience equaling four years

Additional Qualifications:

Must be at least 21 years old to operate a company-owned vehicle and at least 23 years old to operate a 15-passenger van.

Current driver’s license, acceptable driving record, and auto insurance.

Reliable means of transportation for client transportation as needed.

Successful completion of background checks, including criminal record, driving record, abuse/neglect, and fingerprint checks.

Strong communication skills, both verbal and written, with an ability to produce clear, concise reports.

Self-motivated with excellent problem-solving skills and the ability to multi-task in a dynamic environment.

Physical Requirements:

Sedentary work: Exerting up to 10 pounds of force occasionally and/or negligible amounts of force frequently to lift, carry, push, or pull objects.

Repetitive movements of hands, fingers, and arms for typing and/or writing during the work shift.

Occasional walking or standing, with most duties performed while seated.
