Community Outreach Worker

Fairfax Location & Community Involvement - The Community Outreach Worker applies understanding of the experience, language, and culture of the populations they serve to promote healthy living and to help people take greater control over their health and lives. Partners in the delivery of health and human services to carry out the following roles by providing culturally appropriate health education and information, links people to direct service providers, including informal counseling and advocates for individual and community needs, including identification of gaps and existing strengths and actively building individual and community capacity. 
 

Monday - Friday (Some Weekends as needed) Day Time with some flexibility.

Job Responsibilities 

 Provides direct support and advocacy for a multi-disciplinary, hospital-based violence intervention program dedicated to improving access to healthcare services by enhancing prevention and intervention services to patients and families experiencing community violence. 

Provides hospital and community-based support services to patients who come into the emergency room/admitted to Fairfax Hospital that have experienced intentional violent injury and community violence through screening, assessment, crisis intervention, direct service referrals and case management. Facilities communication and networking amongst service providers to ensure service connections that efficient and effective for patients being serviced.  Provides accompaniment for victims of community violence with Fairfax Healthcare Systems as needed  Provides on going intensive case management to identified individuals with recourses that provide access to economic, food, housing, educational, social, and medical supports, and on-going stability.  Provides on-going case management and follow up services post discharges and assist in administering and tracking use of crisis funds for victims of intentional violence  Actively participates in community outreach through meetings, conference, committees, and task forces pertaining to violence and prevention efforts  Demonstrate knowledge of the principles of growth and development of the life span and possesses ability to assess data reflective of the patient’s status and interprets the appropriate information needed to identify each patients’ requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.   Performs additional duties as assigned. 



Additional Requirements 

 Education - Associate Degree or HS Diploma/GED and 2 years of experience
