Medical Center: Community Health Development (Full-Time, Day Shift) -
Job Summary: The Community Health Worker Apprentice is responsible for meeting the standards of apprenticeship by completing the competencies that were established in collaboration with LLUH/SMGC/CHW/Promotores program and approved by the Federal Department of Labor. The CHW Apprentice can provide basic healthcare services, maintain client records, advise clients or community groups on issues, assess individual or community needs for educational or social services, transport clients to appointments, provide educational materials to community members, confer with clients to discuss treatment plans or progress, monitor clients to evaluate treatment progress, refer clients to community or social service programs, advocate for individual or community needs, recommend legal actions, teach life skills or strategies to clients or their families, collect information about community health needs, lead classes or community events. Performs other duties as needed.
Education and Experience: High School Diploma or GED required. Will be required to obtain a certificate of program completion of the Community Health Workers/Promotores training program at Loma Linda University Health San Manuel Gateway College.
Knowledge and Skills: Knowledge and experience working in a multi-cultural setting. Knowledge of community resources appropriate to needs of patients/families and provide referrals for services to community agencies as appropriate. Knowledge of basic medical terminology preferred. Able to read; write legibly; speak in English fluently with professional quality; use computer, printer, and software programs necessary to the position (e.g., Word, Excel, Outlook, PowerPoint). Operate/troubleshoot basic office and patient care equipment required for the position. Perform required patient care activities related to the position. Relate and communicate positively, effectively, and professionally with others. Work calmly and respond courteously when under pressure; collaborate and accept direction. Think critically and manage multiple assignments effectively; organize and prioritize workload. Problem solve and recall information with accuracy. Pay close attention to detail. Work independently with minimal supervision. Distinguish colors as necessary; hear sufficiently for general conversation in person and on the telephone and identify and distinguish various sounds associated with the workplace; see adequately to read computer screens, and written documents necessary to the position.
