Community Health Worker 2

Schedule: Mon - Fri 8 am - 4 pm 

Salary: $ 55,000/yr

The Community Health Worker (CHW) II acts as an ambassador of the community partners (CP) program, rapidly forming close working relationships with key stakeholders, interdisciplinary care teams, accountable care organizations (ACOs) and managed care organizations (MCOs) to promote integrated care. The CHW II supports the CP Team Lead in strengthening programmatic performance through mentorship to staff and participation in special projects as assigned by CP leadership.

As an integral member of the CP team, the CHW II aids in the onboarding of newly hired CP staff through job shadowing opportunities and modeling the expected role, responsibilities, and scope of practice of the Community Health Worker. Serving as a mentor, the CHW II coaches the staff in key core competencies and efficient and effective work processes. The CHW II guides by example, exhibiting exemplary care coordination practices including outreach, engagement, assessment, care planning, transition planning, health and wellness coaching, and social service linkage for MassHealth Members with complex medical and behavioral health needs. The CHW II excels at identifying and addressing gaps in care in a manner that demonstrates the values of person-centered planning, Community First, and Substance Abuse and Mental Health Services Administration (SAMHSA) Recovery Principles. The CHW II is proficient at organizing and coordinating resources and services in response to the Member's healthcare needs across multiple settings, inclusive of both long-term services and supports (LTSS) and social determinants of health (SDOH) needs.

The Community Health Worker II has a range of duties including:

Supports public relations efforts and develop community stakeholder partnerships. Leads or participates in special projects assigned by the CP Leadership Team. Participates in implementation of workflows, processes, and programmatic performance improvement plan as delegated by the CP Leadership Team. Provides, supports, and/or fosters the mentoring, coaching, and training of CHW staff. Updates, maintains, and disseminates key information and resources used by CHWs. Persists in outreach to engage "hard to reach" individuals. Coordinates and documents the completion of the Comprehensive Assessment (CA). Conducts and documents initial and ongoing risk assessment; designs personal crisis management plans, relapse prevention and harm reduction strategies with members who have been identified as behaviorally complex in collaboration with the team. Perform other related duties as required.
