Our hospitals, care sites and clinicians are recognized for clinical and operational excellence. **Toledo, OH** Job Description **Summary of Primary Function/General Purpose of Position** This position will provide direct patient interaction and coordination of services at the direction of a Nurse or Social Worker in the Clinical Services team. Provides an excellent patient experience during the episode of care, using extensive knowledge of patient resources in the local community to provide excellent patient experience. The Community Health Worker may provide ongoing individualized support to the patient when a licensed care manager or transitions nurse has determined the patient does not need intensive assessment and clinical services. **Essential Job Functions** Provides ongoing episodic community-based support and assistance to patients and staff to implement care plan as deemed necessary by the licensed Care Coordination Team. Assist clinical and administrative staff in for preparing patient appointments (pre-visit planning) Act as a resource to patients and families, providing answers to frequently asked questions and suggesting educational materials using Care Coordination Team Protocols Maintain comprehensive progress notes for each patient interaction. Maintains good working relationships with and knowledge of applicable community services. Utilize interpersonal skills and community resources to effectively advocate for individual in the communities served. Maintains required data and documentation and prepares required report for the Care Coordination Team Perform duties with minimal supervision. May be required to occasionally work irregular hours, attend other job-related meetings, and perform other duties as assigned. Collect patient information or reporting of symptoms according to protocol, without assessment or patient direction. Prompt escalation to Nurse and/or Provider managing the patient’s care. Clear understanding of scope of patient services and interactions that require a licensed clinician. Employees may be required to perform other job-related duties as required by their supervisor, subject to reasonable accommodation. **Patient Population** Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. **Employment Qualifications** Required Minimum Education: GED Preferred Education: Associate degree Specialty/Major Healthcare or related. **Licensing/ Certification** Licensure/Certification Preferred Community Health worker certification **Minimum Qualifications** Minimum Years and Type of Experience 1 year experience in community or ambulatory care-based setting with direct patient interaction. Other Knowledge, Skills and Abilities Required Excellent interpersonal communication and negotiation skills. Strong analytical, data management and computer skills. Demonstrate basic knowledge of healthcare and health education across the lifespan in a practice health setting. Ability to work with individuals, groups and families. Familiarity and knowledge of Community Resources. Flexibility to work non- traditional hours. Works well in a Team Setting. Personal computer skills. Experience with database entry, EMR documentation, Power Point preferred and basic Excel skill. Highly organized and detail oriented. Accepts responsibility and follows through on projects and activities. Other Knowledge, Skills and Abilities Preferred Demonstrated success in improving the health of a distinct population of patients in the ambulatory or community setting. Experience in a healthcare related environment working with patients, families, and providers. Ability to review education and resource materials with patients and families/support system.
