The Community Engagement Specialist works collaboratively with the Director of Quality to enlist client involvement, engagement, and participation in ongoing efforts to improve the quality of services.  The role is responsible for the recruitment and facilitation of the Consumer Advisory Board, ensuring consistent communication about upcoming meetings and the importance of providing first-hand feedback regarding the services and care provided at AID Atlanta. 

 

ESSENTIAL DUTIES & RESPONSIBILITIES

Actively guide/refer patients with information and strategies on how to access HIV/AIDS treatment and support servicesFamiliarizes patients with clinic services and staff contacts Answers patient questions and provides appropriate resource information. Directs questions outside of their scope of work to appropriate staff and ensures that patients receive a response. Refers patient questions regarding medications to appropriate staffAlerts supervisors and or other clinic staff of issues impacting patient’s access to HIV/AIDS services Provides encouragement and support to patients. Shares knowledge and experience in accessing services as appropriate Participates in all staff meetings, CQI and program evaluation activities as directed by the Supervisor Seeks guidance from Supervisor or identified health center personnel when experiencing frustration, other strong feelings, or is unsure about what to do in relation to their work Sets appropriate limits and boundaries Assists in the planning and facilitation of monthly Community Advisory Board MeetingsHost and facilitate at least one Consumer Advisory Board meeting per month. Promote Consumer Advisory Board to expand participation and engagement for more meaningful involvement of members of the agency.Attend monthly agency Quality Management meetings and engage in related quality improvement activities.Engage members in quality improvement activities through recruitment and incentive distribution.Collect and compile contact information for members attending Consumer Advisory Board meetings in accordance with agency, state, county, and regulatory guidelines.Support adherence to medical care services through coordinated educational presentations and materials provided to Consumer Advisory Board members.Effectively coordinate with internal and external programs to ensure accurate information about agency programs and services.Attend needed training for essential duties provided under this contract.Attend agency required HIPAA, confidentiality, and PHI training.Educate patients/members about HIV/AIDS and provide information as needed.Educates members of the target population about AID Atlanta's programs and services.Compiles and creates educational information and resource materials in user friendly formats for distribution to members of the target populationMaintain client confidentiality in accordance with federal, state, and local laws and agency policy.

QUALIFICATIONS:

EDUCATION & WORK EXPERIENCE

High school diploma or equivalent requiredKnowledge of the environment and systems in which patients receive HIV/AIDS care services Basic knowledge of computer programs such as Microsoft OfficeBilingual (English/Spanish) strongly preferred but not requiredMust pass criminal background check AHF Commitment
