Founded in 1906, McLeod Health is a locally owned and managed, not for profit organization supported by the strength of more than 900 members on its medical staff and more than 2,900 licensed nurses. McLeod Health is also composed of approximately 15,000 team members and more than 90 physician practices throughout its 18-county service area. With seven hospitals, McLeod Health operates three Health and Fitness Centers, a Sports Medicine and Outpatient Rehabilitation Center, Hospice and Home Health Services. The system currently has 988 licensed beds, including Hospice and Behavioral Health. The hospitals within McLeod Health include: McLeod Regional Medical Center, McLeod Health Dillon, McLeod Health Loris, McLeod Health Seacoast, McLeod Health Cheraw, McLeod Health Clarendon and McLeod Behavioral Health.

Job Summary

Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values. The Community Health Worker will provide assistance with coordination of access to health care services and provider referrals to low income uninsured or underinsured clients eligible for the Access Health/HOP program. Will help to facilitate access to health care by reducing or resolving barriers for assigned and/or referred clients. Responsibilities of the Community Health Worker include, but are not limited to, case coordination, patient education, travel/home visits, and direct follow-up related to accessing and/or retaining health care and supportive services. Must use own car for travel to remote sites, often in rural, isolated areas. Ability and willingness to provide emotional support, encouragement, and motivation to patients.

Work Schedule: Full Time. 80 Hours Bi-weekly

Qualifications /Training: 

Knowledge base of community services and resources. Must be able to handle potential problems related to safety issues for staff and patients in all settings.  Knowledge of medical terminology required.  Basic computer skills (Excel and Word) Good communication skill, such as listening well, and using language appropriately May be required to drive patient’s home. This position will cover the Manning, SC area.

Licenses/Certifications/Registrations/Education:

High School Diploma or GED Willing to obtain BLS Physical Requirements A valid driver's license with acceptable driving record is required and proof of current auto insurance.
