Collaborative Care Behavioral Health Consultant, LCSW or LCPC
Job Description

Coordinates and supports Behavioral Health Care within local Primary Care and Specialty Care Outpatient practices. Coordinates referrals to clinically indicated services. Supports the treating provider by coordinating behavioral health treatment, proactive follow up, alerts the provider when additional treatment options are needed, supports medication management, and facilities communication with psychiatric prescriber. Maintains billing and registry for all patients enrolled in program and uses evidenced based, trauma informed, and measurement based care techniques aimed at complete recovery.

Qualifications

Education & Training:

Currently licensed as LCPC, LCSW-C, or PsyD in the state of Maryland

Work Orientation:

Minimum of five years of psychiatric clinical experience

Minimum 2 years providing psychotherapy including: a) screening for mental health and substance use disorders b) brief, structured intervention techniques c) evidenced based counseling techniques d) working with person’s that have co-morbid physical health disorders

Skills & Abilities:

a) effectively communicate with health care providers as a team

b) effectively engage person’s into a therapeutic relationship

c) working knowledge of differential diagnoses

d) basic knowledge of common psychopharmacology

e) flexibility to work within a variety of healthcare environments and with a variety of healthcare professionals
