Position Summary

 

The Certified Nursing Assistant (CNA), under the supervision of a Registered Nurse or Therapist, performs various personal care services for patients in the home setting using a patient specific care plan to facilitate the patient’s self-care and independence with activities of daily living. Responsible for observing patients, reporting these observations, and documenting observations and care performed.  Performs other duties as assigned.

 

Minimum Requirements

Education           

High School Diploma or GED Equivalent

 

Experience        

One (1) year of personal care experience

 

License/Registration/Certifications       

State certification for Home Health Aide training program and competency and is listed on the South Carolina state registry Valid SC Driver’s License with good driving record CPR certification

Core Job Responsibilities

Adheres to the care plan established by the RN or Therapist; assists patient/family as needed. Notifies RN or Therapist when updates to care plan are needed. Assists patients with personal care activities including but not limited to: bathing, dressing, grooming, skin care, hair care, nail care, feeding, oral hygiene, and shaving. Toilet’s patients and assists with bedpans and urinals; provide catheter care, empty catheter bag, if applicable. Assists patients with use of cane or walker, special eating utensils, special techniques, and equipment for personal hygiene. Document’s care performed as outlined on care plan in the electronic medical record. Reports all changes in patient’s condition (physical and/or mental) and environmental changes to case manager. Assists family members with and provides training on lifting, turning, re-positioning, and transferring patient. Provides emotional support to patients and/or family members. Maintains a safe, secure, and healthy patient environment by following asepsis standards and policies. Participates in 12 hours of in-service training. Performs other job duties as assigned.

  Category I - Position includes tasks that involve exposure to blood, body fluids, and tissues.

 

  Category II -   Position includes tasks that involve no exposure to blood, body fluids or tissues, but employment may require performing unplanned category I tasks.

 

  Category III - Position includes tasks that involve no exposure to blood, body fluids or tissues.  This position would never be required to perform category I tasks.

Ability to withstand stress and demands of an active position Stand, walk, stoop, kneel, bend, and sit for short or extended intervals Lift, and/or move in a push or pull manner more than 50 pounds occasionally to reposition patient whether seated, lying on a bed or leaning against an object Carrying or lifting objects such as medical equipment, laptop, and supplies Ability to walk up the stairs and on uneven surfaces Extensive driving of personal or fleet car Manual dexterity, good eye/hand coordination Full range of body motion Acceptable vision and hearing needed in daily work Ability to talk and hear Ability to read, write and speak English fluently
