Overview
Assurance that each family receives a comprehensive, person-centered treatment program of support while ensuring that each family’s therapy plan is consistently implemented and documented.Oversees all clinical treatment planning.Responsible for completing treatment plans, quarterly progress reports, and all graphing.Collect data on target behaviors and target programming.Prepares and maintains written case notes after all home visits, IEP’s, or direct supervision sessionsMonitors protection of all rights and reports any violations according to company policies, state, and/or federal regulations.Identifies gaps in service provision and/or company procedures and communicates these issues with the appropriate supervisory staff.Conducts self in positive, respectful and collaborative manner demonstrating proactive actions and decision making that foster supportive work relationships and a healthy and safe environment. Communicates effectively verbally and in writing with team members and families maintaining professional conduct and confidentiality.Functions within legal, licensing, company and professional standards in maintaining a therapeutic environment for all people served.Adhere to the Wisconsin Medicaid requirements.Demonstrated knowledge and appropriate implementation of company policies, procedures, and information outlined in the Employee Handbook.Reports any suspected abuse, neglect and/or exploitation as mandated by the state, federal, and/or company policy.Successfully completes all required trainings.Works scheduled hours/shifts as determined by the needs of their caseload and under the direction of their supervisory staff.Assures, through the use of communication systems, that staff have knowledge and information to implement clinical treatment for families.Submits request for the time off and schedule changes to their supervisory staff.Submits verification of accurate daily/weekly time records per policy.Accepts caseload and team assignments per their Direct Supervisor.Collect data on target behaviors and target programming as well as prepares and maintains written case notes after all shifts per policy.Reports any violations according to company policies, state and/or federal regulations.Other duties as assigned/delegated by supervisory staff which may not be specifically provided for in this description, but reasonably fall under this position.Additional Information
KNOWLEDGE, SKILLS, AND ABILITIES:
·Must have BCBA (Board Certified Behavior Analyst) certification
·Demonstrated knowledge, skills, and experience supporting children, adolescents and/or young adults diagnosed with emotional, mental health, neurological and/or developmental disabilities.
·Must have had a valid driver’s license for the past 3 years, valid car insurance and access to reliable transportation.
·Effective working alone or as a member of a team.
·Demonstrates the ability to exercise good judgment and effectively solve problems
