Full-time: Monday thru Friday (Onsite) 

Extensive experience in ortho surgical

This candidate will work collaboratively with ortho and spine surgeons to support the ambulatory and surgical practices.This includes but not limited to pre-op education follow up care. Experience with growing a Joint Commission orthopedic certification program preferred.

JOB SUMMARY

The Clinical Services Navigator will facilitate care coordination from acute to ambulatory and is a valued member of the healthcare team. They collaborate with Physicians, Case Managers, and other members of the healthcare team to facilitate coordination of care and follow-up appointments to the ambulatory setting. Collaborates with Case Managers for guidance with medically and/or psychosocially complex cases. Serves as a resource for care coordination. Maintains current knowledge of available community resources, post-acute care and ambulatory options.

Patient Care: Interviews and advises patients regarding health and illness prevention.  

Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.  Recommends community resources to meet patient and family needs.  

Quality: Collaborates with other departments or disciplines to achieve desired quality outcomes. 

Monitors quality of core processes in department.  Applies the principles of safety and infection control to ensure the safety of all patients.  

Communication: Demonstrates superior interpersonal and communication skills.  

Works within a team to achieve patient and team goals.  Shares and initiates regular and professional communication with co-workers.   Participates in regular staff meetings.   Identifies opportunities to improve processes based on feedback from staff and customers. 

Medical Knowledge: Provides primary health care and performs selective medical services under the direction of practice physicians.  

Obtains complete history and physical data on patients.  Reviews patient records.   Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures needed.  Performs a complete physical exam and records findings.  Establishes presumptive diagnosis.  Orders appropriate laboratory and diagnostic procedures.  Performs independent evaluations and treatment procedures in emergency situations.   Provides monitoring and continuity of care between physician visits.  

The incumbent may be asked to perform additional duties as assigned.

 

QUALIFICATIONS

Education: 4 Year/Bachelor’s Degree preferred.

Experience: 2 years of experience preferred. 5 years of clinical experience with knowledge in the care of disease specific population preferred. Refer to the Life Support Training Policy for additional details.

Licensure: RN required. Certified in specialized area preferred. National specialty certification required, or obtained within 2 years, if applicable to position.

Additional Skills/Requirements Required: Demonstrates effective communication skills to provide patient education. Demonstrates effective communication skills to discuss case management, financial, clinical issues with hospital-based case managers, physicians, outside agencies and facilities, nurses, and other healthcare professionals in the community. Autonomous, self-starter with the ability to make independent decisions. Organized with effective time management skills. Detail oriented with ability to facilitate change and affect positive results. Proficient in computer/web-based tools.
