Clinical Pharmacist - Transplant

Work Schedule: 

This is a full-time, FTE 1.0 Day/Evening, 7:00 am – 3:30 pm / 8:00 am – 4:30 pm, with rotating evening, 1:30 pm – 10:00 pm, and every 4th weekend.  Hours may vary based on department needs You will work at University Hospital in Madison, WI.

Pay:

We are seeking a Clinical Pharmacist - Transplant to: 

Provide medication therapy management services as part of the interdisciplinary transplant team to patients on inpatient and ambulatory services to assure that drug therapy is safe, effective, efficient, and cost-effective. Perform both clinical and distributive tasks necessary to ensure that patients’ medication therapy is safe, effective, timely, and efficient. Provide training and education to Pharmacy Residents and PharmD students through practical and didactic experiences.  Provide patient care to a diverse population including kidney, liver, pancreas, heart and lung transplant medical and surgical patients.

Education

PharmD degree or equivalent experience Required

Work Experience

Prior pharmacy related work experience Preferred

Licenses and Certifications

Licensed or eligible for licensure to practice pharmacy in the State of Wisconsin Upon Hire Required Basic Life Support (BLS) certification Upon Hire Required
