Clinical Pharmacist- ED/Critical Care
**Employment Type:** Full time **Shift:** **Description:** **Essential Key Job Responsibilities** _Appropriately assesses patient data to_ _identify_ _medication-related problems through profile reviews, pharmacy consults and bedside interdisciplinary triage and rounding._ _Participate in medical emergencies and other formal alerts such as trauma, stroke,_ _sepsis_ _and STEMI._ _Reviews and interprets_ _physicians_ _orders to assure that the drug, dosage form, and dose is_ _appropriate for_ _the patient's age, weight, and comorbidities. Checks for drug incompatibilities and_ _allergies, and_ _takes_ _appropriate action_ _when necessary._ _As necessary, consult with providers, nurses and other healthcare professionals involved in patient care_ _regarding_ _pharmacotherapy optimization with intent to achieve_ _optimal_ _patient outcomes across the continuum of care_ _._ _Provides_ _accurate_ _and_ _timely_ _responses to other healthcare professionals and patients_ _regarding_ _drug information questions and facility medication use policies to assure safe and_ _appropriate use_ _of medications._ _Serves as preceptor for pharmacy students/residents and_ _assists_ _with orientation, instruction and training of pharmacy students, interns, pharmacy technicians, residents,_ _pharmacists_ _and other health professionals._ _Assists_ _nursing with medication administration (oral, injectable, continuous infusion titration) when_ _necessary_ _depending on the work assignments and_ _in accordance with_ _regulatory and hospital policies._ _Documents patient care activities including therapy plan,_ _monitoring_ _and any follow-up in electronic medical record as per departmental guidelines._ _Responsible for the completion of all assigned work during the shift, while_ _maintaining_ _the highest standards of quality and safety through the completion of quality control checks,_ _identifying_ _and reporting out medication errors and near misses,_ _identifying_ _process improvements and ensuring_ _appropriate documentation_ _._ _Maintains current awareness and adheres to State and Federal laws, regulations, pharmacy policies, standard operating procedures,_ _formulary_ _and pertinent clinical practice guidelines while providing pharmaceutical care to patients._ _The job summary and responsibilities listed above are designed to_ _indicate_ _the general nature of the work performed within this job. Employees may_ _be required_ _to perform other duties as assigned._ **Minimum Qualifications** **Required Education and Experience** Pharmacy degree (BS or Pharm.D.) from an ACPE approved college of pharmacy **Required Licensure and Certifications** Licensed, or eligible for pharmacist licensure in Iowa; Board of Pharmacy Specialties (BPS) certification required within 3 years of hire; BLS, ACLS, and PALS required within 6 months of hire **Required Minimum Knowledge, Skills, Abilities and Training** Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years hospital pharmacy experience; completion of PGY-2 Critical Care or Emergency Medicine Pharmacy Residency or 3 years of experience in Critical Care or Emergency Medicine is preferred **Our Commitment to Diversity and Inclusion** Trinity Health is one of the largest not-for-profit, Catholic healthcare systems in the nation.
