Job Title: Clinical Pharmacist
Location: Columbia, Missouri
Employment Type: Full-Time

Job Summary:
As a Clinical Pharmacist, you'll lead assigned patient service lines, clinical areas, and therapeutic programs while delivering direct patient care and clinical practice. Your main responsibilities will include designing and implementing stewardship activities, developing and overseeing policies and procedures for drug purchasing, usage, distribution, and control, and participating in pharmacy operations and medication dispensing. Additionally, you'll maintain proficiency in hospital computer systems and medication ordering systems, and provide accurate, safe, timely, and appropriate medication therapy based on patient age and needs.

Key Responsibilities:

Lead assigned patient service lines, clinical areas, and therapeutic programsFacilitate pharmaceutical care services, direct patient care programs, and medication utilization systemsDesign and implement stewardship activities and restriction/surveillance programsTrack and evaluate assigned pharmacy programs for operational, quality, and financial efficiencyIdentify practice-related issues and facilitate clinical research, quality improvement, or provider educationDevelop and oversee policies and procedures for drug purchasing, usage, distribution, and controlAssure pharmacy is an integral part of the healthcare delivery system and facilitate enhancement of servicesDeliver direct patient care and clinical practice, including ambulatory-care and service-based programsWork as an active member of multidisciplinary teams and collaborate to provide patient-centered careIdentify high-risk patients and implement interventions to improve quality and safetyMake appropriate evidence-based, patient-centered medication recommendationsParticipate in the management of medical emergenciesProvide discharge medication review, reconciliation, and counselingProvide pharmaceutical services throughout the medical centerComplete critical patient monitoring and medication therapy reviewsCommunicate effectively with healthcare providers and assure continuity of pharmaceutical careActively participate in stewardship activities and restricted medication programsParticipate in pharmacy operations and medication dispensingOversee pharmacy operations and technician practice/activitiesFacilitate specialty medication procurement, ordering, and dispensingMaintain proficiency and actively engage in education/mentorshipDevelop staff training experiences/competencies and create new teaching opportunitiesIdentify, design, and implement improvements in the medication-use system

Education and/or Experience Qualifications:

Graduate of an ACPE accredited College of PharmacyDoctor of Pharmacy or other advanced degree, preferredCompletion of ASHP accredited PGY-1 Pharmacy Residency, preferredCompletion of PGY-2 Specialty Pharmacy Residency, preferredClinical pharmacy experience (2-4 years) preferred (required if no PGY-2 or PGY-1 residency)

Additional Qualifications:

Knowledge of contemporary hospital/clinical practice and serviceSignificant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicineAble to communicate effectively with patientsDetail orientedLeadership experience preferred
