Clinical Pharmacist

This position is responsible for proving high-quality, direct patient care for patients in a variety of community-based pharmacy settings. The Clinical Pharmacist will contribute medication expertise to the interprofessional healthcare team by providing comprehensive medication reviews and disease state management and answering medication-related questions for patients and healthcare professionals. Furthermore, the Clinical Pharmacist will contribute to development of the department and will serve as a preceptor for residents and students. In addition, this position will staff in the outpatient community pharmacy as needed.

 

 

 

Education, Work Experience and Licensure:

 
Education: Graduate from an accredited School/College of Pharmacy.
Licensure: Maintain licensure as a Registered Pharmacist with the State of Ohio Board of Pharmacy.
Certification: Completion of immunization certification required, completion of medication therapy management certification preferred, and completion of a pharmacy practice or specialty resident (PGY1 or PGY2) preferred and/or ability to obtain board certification within 24 months of hire preferred.

   
Experience: Minimum 2 years as a pharmacist in ambulatory care preferred.
