Clinical Care Coordinator - Radiation OncologyReq #:0000188355 Category:Nursing Status:Full-Time Shift:Day Facility:RWJ Somerset Department:Radiation Oncology Location: RWJUH Somerset, 110 Rehill Ave, Somerville, NJ 08876 Job Overview: Participates in all patient care activities with the Attending Physician as assigned and coordinates the delivery of care. The primary responsibilities include but are not limited to assisting in communications with patients, triaging patient and patient issues, symptom management and teaching patients regarding their disease process and treatment. Responsible for evaluating, planning and delivering nursing care to meet individualized patient needs Qualifications: Education required: + Bachelor s in nursing (BSN) Experience: + Oncology experience preferred + Strong communication and organizational skills + Proficient computer skills Certifications and Licenses Required: + BLS and ACLS from American Heart Association + Active New Jersey Registered Nurse License + OCN required or obtained within 1 year of hire Scheduling Requirements: + Position is for days, 40 hours a week Other Duties: + Responsible for evaluating, planning and delivering nursing care to meet individualized patient needs. Identifies a plan of care; and implements and evaluates the care given. + Collaborates with the care team, utilizing clinical outcome data to support the innovative and cost-effective changes to practice and the service line. Coordinates care given by other team members + Improve patient flow by reducing the time the patient waits in clinic.
