Position Overview:
The Care Coordinator supports individuals seeking services by facilitating same-day access, guiding them through the initial assessment process, and connecting them with necessary resources. This role collaborates closely with both clients and their care teams to ensure a smooth and supportive experience.
Requirements:
Must be qualified in at least one of the following ways: High School diploma or equivalent required Bachelor’s degree or higher in one of the behavioral sciences or related fields from an accredited college or university preferred. Certification as a Community Health Worker or Certification as a Family Peer Specialist preferred. Commitment to promoting diversity, multiculturalism, and inclusion with a focus on culturally responsive practice, internal self-awareness and reflection.Responsibilities:
Facilitate mental health screenings for individuals seeking same-day access. Assist individuals in obtaining services across mental health, social, educational, and healthcare domains. Identify and address barriers to accessing recommended services. Communicate with care teams and providers to ensure continuity of care. Conduct same-day access appointments during scheduled hours. Provide consultation and collaborate with internal and external providers. Work with leadership to develop and improve processes for supporting families in accessing services.Fraser Offers:

Pay: The starting pay range for this role is $22.60 - $23.00 an hour depending on qualifications.
