Job Title: Chronic Care Manager (LPN)Job Description We are seeking a dedicated Chronic Care Manager (LPN) to provide one-on-one attention via telephone calls to patients with two or more chronic diagnoses. The Chronic Care Manager will collect patient health information to identify risks, provide education, and encourage compliance with treatment. This role involves creating a patient-specific Care Plan that provides goals and interventions to implement, monitor, and support the needs of the patient as defined by the Primary Care Provider (PCP). The Chronic Care Management (CCM) program aims to promote the highest level of quality care for patients. Responsibilities + Provide initial risk assessment gathering the clinical elements necessary to determine a patient-specific Care Plan. + Work with patients to plan and monitor their care as determined by the PCP. + Promote compliance with PCP/Specialist office visits. + Promote compliance with medication. + Act as a patient advocate, liaison, and information resource. + Provide chronic care education for chronic/complex conditions as determined. Essential Skills + LPN certification required. + Must have high-speed internet (not satellite internet or hotspot). + Proficiency in Microsoft Outlook and Excel. + Familiarity with documentation in Electronic Medical Records (EMRs). + Self-motivated and able to work in a remote environment. + Passion for helping others. Additional Skills & Qualifications + Experience in chronic care management. + Strong communication and organizational skills. + Ability to work independently and as part of a team. Work Environment This position is 100% remote. You will need a reliable high-speed internet connection. The role involves frequent telephone communication with patients and documentation in EMRs.
