**Join our compassionate team as a Certified Nursing Assistant (CNA) in the Transitional Care Unit, where you'll provide essential, hands-on support to enhance the daily lives and well-being of our patients!**
Shifts: Typically, 2p-10p & 10p-6a
**Transitional Care Unit (TCU):** The Unit is a patient focused unit bridging the patient care journey from inpatient care services to their next long-term living environment. TCU provides a multidisciplinary approach to nursing care, aligning a long-term care experience in a hospital-based unit, leading to a well-rounded experience for our patients & team members alike!
**Responsibilities:**
+ Provide direct resident care as directed by RN/LPN, including activities of daily living (e.g., bathing, feeding, dressing).
+ Document resident care and safety checks according to procedures.
+ Assist in unit tasks like passing meal trays, responding to call lights, and transporting residents.
+ Report equipment malfunctions and ensure cleanliness of resident rooms and department areas.
+ Perform routine tests (e.g., weights, vital signs) and assist with intake and output documentation.
+ Communicate effectively and professionally with residents, staff, and families.
+ Actively participate in feedback, learning, and unit improvement activities.
+ High school diploma or equivalent preferred.
+ Knowledge of body mechanics and transfer techniques / long term care experience preferred
+ Ability to effectively communicate both orally and in writing. **Licensure, Certifications, and Clearances:**
+ Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) within 30 days
