POSITION TITILE: Certified Nursing Assistant/Medical Assistant Non-Exempt Position
RESPONSIBLE TO: Nurse Care Manager Supervisor
MAJOR DUTIES: The CAN/MA plays an important role in patient care by supporting the providers and patients throughout their entire visit and is considered an integral part of the team. The CAN/MA role will include both medical and case management of clients by working closely with all other staff members and medical providers. Duties will include:
Preparing patients for standard workflow which includes taking and recording patient’s vital signs (blood-pressure, pulse, temperature, weight, and respiration), completion of screenings, medication reconciliation and basic triaging of patients. Assisting in making sure the exam room is properly stocked and set up and cleaned between visits. Obtaining routine labs per provider orders, including collecting and processing lab samples to send out to the laboratory, appropriate POC testing, and venipuncture. Assisting providers and care team with both internal and external patient medical and substance use/mental health referralsFollowing up on patient referrals and treatment.Receive, document and direct patient phone calls in a timely manner according to clinic workflow and protocol. Assist in scheduling appointments and completing prior authorizationsContact pharmacies to assist with medication refills per provider requestPrepare supplies and room as well as assist providers for basic procedures/examsUnderstand and maintain proper infection control management in the clinic per State and Federal regulatory requirements. Document intake paperwork and other relevant information of patients into the EHR Completing charting requirementsTravel between and set up MAT clinics at clinic sites as scheduled. Completing any other care team duties delegated by supervisor and/or providers.
REQUIREMENTS OF THE POSITION:
High school diploma or equivalent. Current CNA or MA credential in the state of Washington;.Current BLS CPR/1st Aid certification required;Demonstrate understanding of substance use disorders/opioid use disorder and medication assisted treatments;; Have an understanding and experience with patients with substance use disorders is preferable. Ability to use clear, concise and effective written and oral communication and comply with agency policies when sharing, documenting relevant patient care data. Knowledge of community/health resources;Must have the ability to read labels and distinguish between colorsAbility to communicate clearly both verbally and in writing;Ability to drive between work sites throughout the work week;Current unencumbered driver license and proof of current car insurance, with the ability to become an approved fleet driver;Must pass a pre-employment drug screen, TB test and background check.Great attendance;Ability to work well with others.
SUPERVISORY CONTROLS:
Candidate is under the direct supervision of the Integrated Care Supervisor.
GUIDELINES:
CNA must meet the requirements delineated by Lifeline Connections. All employees will receive trainings as required by department and agency.
SCOPE AND EFFECT:
The scope of activity is to support, encourage and motivate patients who have addiction and co-occurring mental health disorders to ensure that they receive comprehensive integrated health care. The candidate will work with the integrated team to help facilitate problem resolution with appropriate support and resources.
COMPLEXITY:
Under supervision, the candidate supports clients who differ widely in age and socio-economic status and who may possess a variety of chronic and serious social, behavioral, physical, and psychological problems and who are intoxicated and may be experiencing withdrawal from substances.
PERSONAL CONTACTS:
Contacts are with clients from all backgrounds, floor staff, treatment team, RN, MD, hospital social workers and medical staff. The candidate may also need to communicate with employers, family and friends of clients. Also, may occasionally come into contact with law enforcement, lawyers, parole officers and other court officials. All personal contacts are carried out in accordance with federal and state statues, laws and regulations dealing with the confidentiality of alcohol and drug dependent client records.
PHYSICAL DEMANDS:
Must be physically able to walk, bend, stoop, stand on feet for long periods of time, twist, kneel, reach, squat, pull, and lift heavy patients or objects.
WORKING ENVIRONMENTS:
Most working hours are spent indoors, in close proximity to clients.
TRAVEL
Travel between office locations will be scheduled in advance and the employee will use their own vehicle for transportation
Salary: $19.75 - 24.75/hr DOE
