Role includes assessment of patient's status, evaluation, coordination of on-going needs, and planning of care and delivery of services, appropriate implementation of community resources through proper discharge planning functions, and monitoring of quality of service throughout the case management process. Performs direct Interqual/M&R utilization review on all acute admissions. **Essential Key Job Responsibilities** 1. Complies with safety and emergency procedures. 2. Complies with Hospital/Department/Unit policies. 3. Documents activities consistent with Hospital/Department/Unit Guidelines. 4. Performs critical/frequent procedures or services. 5. Provides care/services based on identified needs. 6. Uses required equipment for providing care/services. 7. Consults with Hospital/department resources. 8. Demonstrates professional responsibility as per job classification. 9. Demonstrates Core Values Guest Relations/Guest Service Standards. 10. Employees may be required to perform other duties as assigned._ **Qualifications** **Required Education and Experience** Graduate from an accredited school of nursing. **Required Licensure and Certifications** RN:KY **Required Minimum Knowledge, Skills, Abilities, and Training** Prefer 2 years of recent varied acute hospital experience. Strong clinical background with a broad range of clinical experience, and the ability to utilize data and regulatory information in the decision making process. Adequate conversational English is required in order that the individual be able to take direction from management, understand how to complete job tasks, communicate adequately with patients and co-workers as necessary, understand and follow safety guidelines, and understand the Associate Handbook and all hospital policies. **Pay Range** $30.47 - $44.19 /hour We are an equal opportunity/affirmative action employer.
