67729BRAuto req ID:67729BRDepartment:34655 - Case Mgmt/Utlization Review Job Description:RN Case Managers apply the Case Management body of knowledge defined by the Commission for Case Management Certification to assess, plan, implement, coordinate, monitor, and evaluate options and services required to meet assigned hospital patients' health and service needs. Case Managers participate in and drive the utilization review process for the hospital by assessing resources, evaluating healthcare treatments, and negotiating the most cost-effective options available to promote quality and cost-effective interventions and outcomes for patients within a continuum of care. They serve as a liaison who collaborates among physicians, patients, family, appropriate ancillary departments, and external agencies. They develop and implement comprehensive plans by interviewing the patient, family, medical team, and others to assess potential problems and prevent over or under-utilization of resources. They negotiate internal and external services to meet the needs of patients, utilizing community and in-house resources for optimal patient outcomes. They educate patients and families to assist in understanding and carrying out medical recommendations post-hospital discharge. This work aims to improve the individual's health and reduce preventable emergency department visits, hospital admissions, and readmissions.Location:Brooks Memorial Hospital Recruiter:Vanessa BorowiakTitle:Case Manager RNMinimum or Preferred Qualifications:Education/Licensure: Current and unrestricted NYS Nursing license required. NYS Hospital and Community Patient Review Instrument (H/C PRI) Certification within 90 days. New York State Department of Health SCREEN Certification within 90 days Experience: Requires at least three years of recent clinical experience in acute care setting. Experience in discharge planning, case management, utilization review, and clinical pathways preferred.
