As a Case Manager, you will provide support to clients of the Family Ties program and help to hold the over-arching vision of Seneca Family of Agencies. Family Ties services are provided over a continuum of care that includes case management services to kin caregiver families designed to provide substantive services that empower kin caregivers and youth to make lasting gains and assistance with obtaining legal guardianship. The Family Ties program embodies Seneca’s core values of providing strengths-based, family focused, individualized services at the times and in the locations that best meets the kin caregiver and their family’s needs.  It is the responsibility of the Case Manager to ensure the most comprehensive and effective services are provided to kin caregivers and their family’s in the community and to ensure those services are documented in a way that demonstrates compliance with all county, state and federal regulations.

 

ABOUT FAMILY TIES

Seneca's Family Ties is a collaborative program with San Luis Obispo County Department of Social Services. Family Ties is an intervention system of care that provides peer to peer supportive services to relatives or near-kin caregivers who are caring for relative children that could have been at risk of dependency or have become dependents of Child Welfare or Probation. Services provided can include case management (regular visits, formal support, emergency funds, referrals to in-house services), non-case management (access to support groups, parenting/psycho-educational classes, and supplemental food support with bi-monthly food box distribution), legal guardianship assistance, and a recreational team.

Responsibilities Provide assessments and formulate case plans for caregivers and families.Provide consultation and care plan coordination to families.Work effectively and collaboratively with a variety of public/private providers.Maintain appropriate record keeping and reporting requirements and assist in data collection for further program development and research.Present information about Family Ties at community events, trainings, and other county meetings.Develop and sustain support groups for relative caregivers within the community.Advocate for clients to access community resources and identify areas of need in the community and work collaboratively to meet these needs.Offer direct services to client and family services such as procurement of legal services, emergency assistance, making appointments, court advocacy, etc.Facilitate Support Group meetings and other related support group activities to increase knowledge and self-help in the client group.Liaison between Family Ties Program and Social Workers at DSS offices and attend designated meetings at DSS.Maintain strict confidentiality of client and business operations information. REQUIRED

Education/Experience Requirements:
Bachelor’s degree At least 21 years of age.
TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements.
Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policy.
Proof of up-to-date COVID-19 vaccination.

PREFERRED

Experience working with children/youth and families with complex and enduring needs.Experience implementing family-focused services.Supervisory experience and/or program management experience preferred.
