With a staff of approximately 450, reaching peaks of nearly 600 with seasonal hires, a budget of $54.3 million, and a current governing board of 27, this venerable, fully-accredited, independent 501(c)(3) nonprofit addresses some of the most pressing community needs – homelessness, generational poverty, immigration, inequality, hunger, aging in isolation, and communicable disease -- to name a few – with compassion and services of highest quality. **PROGRAM & POSITION SUMMARY** Under general supervision from Program and Division Leadership, this [NON-SUPERVISORY POSITION] position functions as a collaborative team partner supporting the daily oversight and function of the case management team, reporting directly to the Program Leadership addressing client needs. Serving families with associated disabilities, low income and disabled people who may be experiencing homelessness or unhoused. In addition to these duties/responsibilities, the successful incumbent in this position demonstrates experience initiating and sustaining a high level of accountability resulting in positive outcomes, is mission/housing first-focused. Housing support serves the hardest to help families in San Francisco's homeless ess community. Senior Case Manager will support and advise the Program in planning, managing, evaluating service delivery, operations, and processes. The Senior Case Manager responsibilities include but not limited to: collecting data sets and systems, identifying trends, and preparing reports and recommendations. incumbent will also be responsible for supporting policies and procedures, assisting with tracking and evaluating operational and program performance, and identifying and resolving any issues. The SCM, should be able to provide expert guidance to optimize systems, procedures, and processes, evaluate operational and program effectiveness and develop solutions to make improvements or address performance issues. An outstanding SCM should have strong problem-solving skills. This position acts as a welcoming and knowledgeable portal to accessing services provided by Catholic Charities. The SCM along with Program Leadership determines the most appropriate services to stabilize the family's housing situation. Maintaining professional boundaries/distance, codes of conduct, objectivity and ethics. The SCM will work extensively with the case management staff and other collaborative partners to ensure clients are successful. The Program Leadership is responsible for staff supervision. SCM assumes the role of over Program Services delivery. Assist and supports, team development/training, implementing, evaluating program policies, procedures and standards; determining program service levels and enhancements as required; developing and monitoring program in conjunction with program leadership and performs related work as required. **ESSENTIAL DUTIES & RESPONSIBILITIES** Housing First model and any aspects related to the aforementioned. + Lead Case Manager participates in all programs and activities for staff and volunteers and supports other case managers who conduct case management under the direction of Program Leadership. • The Lead Case Manager provides leadership to program's staff and supports the Program Leadership with various programmatic tasks to ensure Performance and Quality Improvement (PQI) standards. • Providing support and guidance to colleagues regarding service delivery best practices including making effective and appropriate referrals and providing on-going support that helps participants meet their goals• • Ensuring that Council on Accreditation [COA] standards are utilized through example and training • Conducting and supporting individualized needs assessments [Client Services Plans] and collaborating with clients to develop permanent housing plans that address any and all barriers to obtaining and maintaining stable housing. • Providing on-going case management support to assess progress and ensure housing plan goals are met or adjusted as necessary • Coordinate the integration of social services/ case management functions into the participates planning process. • Keeps detailed records of every case, every referral and every contact with other agencies on behalf of the clients. • Help clients work through urgent needs. Enable clients to take responsibility for their recovery, acting as an advocate and a facilitator. • Communicate all information to Program Leadership as requested/required and keep detailed records of these meetings.• Network with other agencies to stay informed about services and resources. • Refer clients to another program when appropriate. • Follow up with other agencies that have made commitments to assist clients with their needs • Respect confidentiality and HIPPA requirements at all times, in public, committee meetings, consultations, as well as during off-duty hours outside of work. • Remain in contact with client until the client's needs are met and the case is closed. • Provides daily oversight to the case management services delivery to include, but not limited to, guidance related to documentation, maintaining compliance with regulatory standards, appropriately managing caseloads, and coordinating services + Assistance with benefits?Assistance with obtaining necessary personal identification and records + Independent living and money management skills + Employment assistance and referral/linkages + Housing referral, location, advocacy and application assistance + Referrals and linkage to community providers for health, mental health, and substance use services + Admission process and provide an orientation to new clients utilizing agency intake procedures. + Carry a caseload of approximately 8 -16 families clients assigned by Program Leadership for special needs, higher acuity and crises, providing full case management, advocacy and housing first guidelines. • Serve as interim case manager during absences [vacation and personal leaves up to 30 days] **DISCLAIMER:** The above statements are intended to describe the general nature and level of work being performed by people assigned to this classification.
