Case Manager II or III - Medical Services (Jail-MAT)

Medical Services is seeking a Case Manager to provide case management for individuals with mental illness, substance use or occurring disorders and their families who demonstrate a need for active case management services. The Case Manager coordinates services and/or resources needed by the client to support their basic needs and enable them to remain and function optimally in the community and/or Jail. Works as part of the SA Team to coordinate services for clients enrolled in the MAT programs. Also provides discharge services and active follow-up engage the client in appropriate services. This team works in the 5 clinics located in Pulaski, Radford, Giles, Floyd, and Blacksburg, as well as the local jails. Starting Hourly Rate:

Case Manager II: $17.56

Case Manager III: $18.44

 

Education Requirements:

Case Manager II:

Bachelor's Degree in Education, Social Work, Psychology or Human Services related field defined by the VA Board of Counseling is required. At least one year experience in a setting working with individuals with disabilities, mental health needs, substance use or Developmental Disabilities preferred. Registration as a QMHP or trainee is required. Must maintain QMHP status and all required trainings/CEUs. Previous Case Management experience is preferred.

Case Manager III:

Bachelor's Degree in Education, Social Work, Psychology or Human Services related field defined by the VA Board of Counseling is required. At least one year experience in a setting working with individuals with disabilities, mental health needs, substance use or Developmental Disabilities preferred. Registration as a QMHP is required. Must maintain QMHP status and all trainings/CEUs. Previous Case Management experience is preferred. CSAC is preferred.

Additional compensation will be considered for any new employees who speak multiple languages
