Want to make a difference in the lives of others? Same Day Access is looking for a motivated case manager to join their team. The case manager works within the Same Day Access (SDA) system to provide case management for those individuals with mental illness, substance use or co-occurring disorders and their families who attend the SDA process, and demonstrate a need for active, short term case management services (90 days or less) such as linkage to resources and coordination with other agency programs. The case manager coordinates services and/or resources needed by the client to support their basic needs and enable them to remain and function optimally in the community. Also provides discharge services and active follow-up to engage the client in appropriate services. This position provides triage and coordination among the SDA team and clients who have come through SDA.

 

Hourly Rate:

Case Manager I: $16.73

Case Manager II: $17.56

 

Education Requirements:

Case Manager I:

High School Diploma with at least one year of experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities is required.  Bachelor's Degree in Education, Social Work, Psychology, or Human Services related field is preferred. Experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities is helpful.

Case Manager II:

Bachelor's Degree in Education, Social Work, Psychology or Human Services related field is required. Registration as a QMHP or trainee is required. Must maintain QMHP status and all required trainings/CSUs throughout employment.
