Case Manager I or II - Medical Services

Description 

The Case Manager I or II for Medical Services, is assigned to Psychiatric Services Team. The position provides case management for individuals with mental illness, substance use or co-occurring disorders and their families who demonstrate a need for active case management services. The Case Manager coordinates services and/or resources needed by the client to support their basic needs and enable them to remain and function optimally in the community. Works as part of the SA Team to coordinate services for clients enrolled into the MAT program. Also provides discharge services and active follow-up to engage the client in appropriate services. Attends treatment team meetings, Family Assessment and Planning Team (FAPT) meetings, and maintains contact with referral sources (i.e.: Probation, DSS, etc.) as needed. 

MAT SA Case Manager I-$34,798-Depending on Experience

Bachelor's degree in education, social work, psychology, or human services related fields is required with at least one year of experience or equivalent as listed in licensure regulations, CSAC preferred, case management experience preferred.

MAT SA Case Manager II-$36,524-Depending on Experience

Bachelor's Degree in education, social work, psychology, or human services related fields is required with at least one year of experience or equivalent as listed in licensure regulations. CSAC preferred, case management experience preferred. QMHP required or must be obtainable.

Qualifications 

Thorough knowledge of case management/social work practices and resources available to help individuals with serious mental illnesses, substance use disorders and individuals with co-existing disorders.  Verification of Qualified Mental Health Case Manager Status via checklist  Working knowledge of Medicaid guidelines as they pertain to State Plan, and Clinic Options.  Basic knowledge of human rights, ethical, and legal concerns as they relate to providing case management clinical services.  Demonstrated ability to communicate clearly and concisely (written and oral).  Basic working knowledge of computers and word processing.  Ability to initiate work, work independently and/or with other team members and resources. Ability to prioritize actions, problem-solve, think critically and improvise; especially helpful for crisis management and creative implementation of consumer plans within guidelines and regulations. Demonstrated understanding of issues around cultural competence and finds value in a commitment to diversity and inclusion.  Knowledge and understanding of best practice treatment modalities for serious mental illness, intellectual and developmental, substance use and co-occurring disorders to include behavioral supports and interventions.  Knowledge of Substance Abuse Block Grant requirements.  Must possess a valid driver’s license (state of Virginia, unless otherwise approved by agency) with an acceptable driving record. Must maintain acceptable record throughout employment.  Must have an acceptable criminal history background and child protective services check upon hire and maintained throughout employment.  Must have acceptable drug screen results upon hire. 

Education/Experience Required: 

Bachelor's Degree in education, social work, psychology or human services related field is required with at least one year experience in a setting working with individuals with disabilities, mental health needs, substance use or Developmental Disabilities preferred.  CSAC is preferred.  Previous Case Management experience preferred.
