Case Manager/ Career Counselor
The Opportunity
The Case Manager takes an active role in assessing and assisting residents in the program to meet their Individual Treatment Plan goals in the areas of educational/vocational planning and employment goals. The Case Manager works as part of a team that develops a milieu where youth take responsibility for their behavior and decisions.
Responsibilities
+ Develop a network of Vocational training and job placement resources
+ Assess residents for basic skills in education and vocation and coordinate, and monitor the educational planning for all clients.
+ Supervise and implement tutorial services and mentoring for residents
+ Conduct/coordinate employment and education workshops as needed.
+ Provide aftercare services for former residents.
+ Assist residents in obtaining necessary documentation and health insurance.
+ Perform Step Up to Success (SU2S) assessments, input all data/information and assign SU2S tasks. In addition, monitor client's progress in SU2S and attend meetings.
+ Enter in case notes in the HMIS system.
+ Conduct intakes for residents into the program when needed.
+ Participate in monthly case reviews for each resident.
+ Assess the youth for services and make all necessary referrals in a timely manner.
Qualifications
+ Bachelor's Degree in Human Service related field required, Master's degree preferred.
+ Minimum of 2 years of experience with adolescence in a residential environment
+ Excellent verbal and written communication skills.
Basic computer skills with MS Office
+ Valid and clean driver's license

Salary: $19.21/hr
