This role involves creating and implementing individualized service plans, facilitating group support, and coordinating referrals to promote client recovery and well-being in alignment with FLACRA’s mission, vision, and values. Required Qualifications: + Associate’s Degree in Health or Human Services. + Bachelor’s Degree preferred. + Valid NYS Driver’s License. + Pursuit of Qualified Health Professional (QHP) status is encouraged. + Experience in a Substance Use Disorder setting preferred. + Job Requirements: Key Responsibilities: + Develop and implement individualized client service plans, ensuring plans are initiated, followed up on, and modified as needed. + Maintain accurate and timely documentation in compliance with FLACRA, OASAS, and agency policies and procedures. + Facilitate community meetings and group support sessions, designing and coordinating groups based on client needs. + Provide one-on-one counseling to residents, helping them develop interpersonal, problem-solving, and decision-making skills. + Respond appropriately to crisis and emergency situations to ensure the safety and well-being of clients. + Complete discharge activities in accordance with agency policies and procedures. + Assist in coordinating treatment referrals and ensure the facilitation of necessary services. + Actively participate in agency staff meetings, in-service training, and supervisory sessions. + Foster a team-oriented environment, demonstrating flexibility, cooperation, and collaboration with coworkers and supervisors. + Monitor client progress regularly, addressing needs as they arise. + Exhibit professionalism by delivering quality care with respect and dignity for all clients. + Perform additional duties as assigned to meet program needs. Skills & Experience: + Strong written and verbal communication skills. + Proven ability to communicate effectively and work collaboratively with culturally diverse individuals, staff, and community service providers. + Familiarity with the local behavioral health system. + Ability to multitask, prioritize effectively, and remain calm in crisis situations. + Proficiency in electronic record-keeping systems (E-records).
