Case Manager Shelter- Ana's Place, 4380 Bronx Blvd, Bronx, New York, United States of America Req #1129 Thursday, December 12, 2024 Title: Case Manager Program: Ana’s Place Salary: $38,000 - $42,000 per year Program Description: Ana’s Place is a 108 bed 24/7 emergency shelter for homeless men diagnosed with mental illness and/or substance abuse. Our comprehensive services include case management, individual and group counseling, recreational activities and housing placement assistance. Additionally, psychiatric services are provided Monday through Friday, staffed by a psychiatrist, nurse practitioner, registered nurse, and a licensed practical nurse. Essential Duties and Responsibilities: The essential duties of the Case Manager include but are not limited to the following activities: + Conducts intake and assessments, including psycho-social evaluations + Provides case management and counseling services and assists clients in the creation and adherence to their Individual Living Plans, identifying short and long-term objectives to be achieved. + Inputs all client information and updates in a timely fashion into the CARES system. + Works collaboratively with the clients and other staffs to identify and obtain financial entitlements, housing and other concrete services. + Maintains accurate and up to date written client documentation as required and in accord with city and state oversight agency guidelines. + Provides escort services to clients as needed. Qualifications + High School/GED Diploma and experience as described below OR Associate's Degree + One to two years (1-2) year of experience working with homeless adults, substance use and/or mental health populations. + Teamwork, Organizational, Interpersonal, Communication, Computer, Bilingual a plus + Knowledge of case management software as well as a proficiency in Microsoft Office Suite Other details + Pay Type Hourly + Min Hiring Rate $38,000.00 + Max Hiring Rate $42,000.00 + Required Education Equivalent Experience + Shelter- Ana's Place, 4380 Bronx Blvd, Bronx, New York, United States of America <
