Heritage Ministries is in growth mode and currently seeking a Case Manager to join our team! The Case Manager is responsible to act as a liaison for the residents’ care by evaluating their physical condition and environment, identifying areas of concern, and completing DOH paperwork as necessary. Responsibilities:

Responsible for, executing, through direct performance and coordination, the services and functions stipulated in DOH regulations. Additional education or experience in assisted living is desirable and will need to be approved by DOH to perform the Assisted Living Case Management Responsibilities based upon DOH regulations.  Demonstrated understand and experience with computer application, typing, filing, and knowledge of office procedure. Accuracy and attention to detail required. An effective ability to read, write, and speak the English language is required.Ability to interact appropriately and professionally with staff, residents, vendors, volunteers, and the public in person and via the telephone.Ability to prioritize and coordinate tasks and assignments.Must be able to meet health assessment requirements, including required testing and health documentation.Ability to lift over 60 pounds at various times as necessary.  Ability to stand, sit, walk, bend, stoop, squat, twist and engage in repetitive motion for extended periods of time.Ability to meet deadlines and handle stressful situations.
