We are seeking a Case Manager to join our team in Wilkes Barre, PA.

 

Position Details

Perform phone triage, verbal de-escalation, and assessment to determine appropriate response to the initial referral Core responsibilities will include providing short term case management and coordination of services and community resources Assist individuals in obtaining and maintaining basic living needs and skills related to housing, food, psychiatric and medical care, transportation, socialization, education, and employment, in a manner that is clinically appropriate Act as a liaison with family, service providers, and community supports and services Be the subject matter expert on community resources and service delivery systems Provide support within three days of the initial crisis event to ensure appropriate linkage and coordination has been started Provide necessary coordination to ensure appointments and linkages to resources occur within 7 days of initial crisis event Respond to urgent/emergency (non 911) calls and provide crisis interventions, risk assessment, safety planning, and make linkages to appropriate levels of care of community resources Quickly establish therapeutic relationships with the individual and/or family being served utilizing trauma informed care approaches and recovery orientated best practices Develop short term goal plans in conjunction with the individual and/or family Educate and provide support to the individual and/or family on their rights and how to navigate the mental health and legal system Complete all paperwork in a timely and accurate manner Attend activities that help educate the community and surrounding service system on the MCRSS program and the benefits it can provide to the individuals, families, and community Demonstrate trauma informed care practices and recovery-oriented approaches in all interactions Demonstrate culturally competent skills in daily interactions with team, individuals served and stakeholders Follow all Merakey policies including but not limited to standards of conduct, confidentiality requirements, standards of professional behavior Other duties as assigned

Earn: $21 per hr.
