Position Summary:
Person in this position will be primarily responsible for managing a small caseload of clients with disabilities that are homeless to ensure they transition to independence. This will include case management activities and care coordination, health education, interdisciplinary collaboration, coordination and consultation, and administrative duties. The Case Manager will work closely with the Disability Advocate and other housing providers to ensure a successful transition into permanent housing. This person will be responsible for analyzing data, creating reports, and for the monitoring of Quality Review and compliance of WestCare s contractual obligations and or agreement. Additionally, they will train staff to produce and maintain quality client files. Essential Job Functions include those listed below. Other duties may be assigned as needed
Essential Job Functions:
Builds and maintains relationships with property owners/managers to secure units for leasing; Meet with each client at least once weekly to provide support and assistance towards achieving each Individualized Service Plan; Intake and assessment of incoming participants to determine eligibility and appropriate services; Act as advocate on behalf of the client to improve access to needed community services. Responsible for ensuring that the clinical records are maintained in compliance with agency, state, and other rules and standards, guidelines and regulations; Conduct quality assurance reviews of client files on a daily basis; Heavy documentation in the Clinical Data System (CDS) and in the Homeless Management Information System(HMIS) data entry; Assist, train, and monitor data collection in the CDS and HMIS systems. Provide training and presentations to staff regarding the Housing and Disability Advocacy program (HDAP) client files, reports, and grant outcomes; Develop, implement, and train on the standards for client file inspections and program reporting; Assist management with daily, weekly and monthly reports as requested; Monitor the system, procedures and standards of the program, informing management when deviations occur; Ability to maintain relationships with referring and supporting agencies, peers and represent the agency at various community meetings as requested; Ability to develop and maintain internal and external partnerships Work closely with Accounting to ensure timely and accurate invoices; Attend scheduled meetings and presentations with referral agencies; and Perform any other duties as assigned.
